PERSONAL DATA PROTECTION AND COOKIES NOTICE
This document outlines the principles of personal data protection at the private practice VILEA, s.r.o., located at Kapisztóryho 5, 940 01 Nové Zámky, Company ID: 36848841 (hereinafter referred to as the "controller"). It governs how personal data is collected, processed, stored, and handled in connection with the provision of healthcare services.

How to apply your rights:
- in writing – by post to the controller's address
- in person – at the controller's registered office,
- by e-mail at: heni.vida@gmail.com
Each request will be recorded and processed without undue delay, no later than one month from receipt. In case of an extension, we will inform you along with the reason.
You will receive a response in the same way the request was submitted, unless you request otherwise.
2. Legal basis for processing
We process personal data on the basis of:
- Legal obligation – e.g. patient records, maintaining medical documentation.
- Contract performance – e.g. providing healthcare based on an agreement.
- Legitimate interest – e.g. property and personal protection (CCTV).
